A Monmouth County, New Jersey, woman has been indicted for allegedly submitting more than $500,000 in bogus invoices to her health insurance company.
A state grand jury indictment alleges that from March 2011 and March 2013, 40-year-old Middletown resident Donna Dzienisiewski submitted 107 invoices to Horizon Blue Cross Blue Shield of New Jersey. The invoices reflected about $502,000 in reimbursable expenses for medical services.
She allegedly visited the doctor on only six of those occasions and incurred about $2,600 in expenses.
Authorities allege Dzienisiewski received about $141,000 from the insurance company.
She faces counts of health care claims fraud, theft by deception, identity theft and falsifying records.
The most serious of the crimes carries a maximum 10-year sentence.
It was not known if she had retained an attorney.
